Patients are required to bring the following when booking on Pre-Admissions:
- Pre-Admissions Form
- Patient Medical Aid Card
- Patient ID
- Main Member ID
- Doctor Referral Letter
- Proof of Medical Aid
- Authorisation
Patients will also need to sign the admission contract at the hospital.
